Now two ways to get your 2011 Win/Loss Request Form
1) Win/Loss Statements now available on myStation.com
New & easy for 2011 statements: You can now download online and print your Win/Loss Statement right from your computer.
Log into your myStation.com account
From the Home tab click on the green "Print Your Win/Loss Statement" on the left side of the page. It's that easy!

2) Manually request a Win/Loss form
Win/Loss Statements by manual request are available to be mailed to you by downloading and submitting our 2011 Win/Loss Request Form*. Win/Loss Statements will be sent out in the order they are received once the current tax year ends.
Completed forms can be brought to the Boarding Pass Rewards Center at any Station Casino, Fiesta Casino or its subsidiaries. Please have valid photo identification with you when dropping off the form. The forms sent via mail must be completed in their entirety and signed in the presence of a Notary Public. Station Casinos LLC must receive the original form sent to us via mail or in person before we can proceed. No photocopies or facsimiles will be accepted.
